The first time Yuuji died was when Sukuna took out his heart. In this episode, when Yuuta stabbed Yuuji, there is a scene of Sukuna lifting his head as if noticing that Yuuji's heart has stopped beating. I keep thinking that if Sukuna willed it, he could have manifested right then and there when his vessel/cage died. It was a gamble to kill Yuuji and subsequently heal him with RCT (reverse cursed technique).
